Aim: To evaluate oncologic and functional outcomes of salvage partial laryngectomy (SPL) as an organ-preserving strategy in patients with recurrent laryngeal and hypopharyngeal squamous cell carcinoma following radiotherapy or chemoradiotherapy. Study Design: Retrospective cohort study. Methods: A retrospective review was performed of patients undergoing SPL between July 2014 and July 2024 at a tertiary care centre. Surgical approaches included open partial laryngectomy (OPL), transoral laser microsurgery (TLM), and transoral robotic surgery (TORS). Primary outcomes included disease-free survival (DFS), overall survival (OS), local control, tracheostomy and feeding tube dependence, and swallowing function assessed using the Functional Oral Intake Scale (FOIS). Results: Twenty-three patients were included in the analysis. Median follow-up was 3.7 years. Local recurrence occurred in four patients, yielding an overall local control rate of 82.6%. No patient required completion of total laryngectomy. Mean disease-free survival was 2690 days (approximately 7.3 years); median DFS was not reached. At the last follow-up, 17 patients (73.9%) were alive. Median tracheostomy decannulation time was 12 days, and median time to initiation of oral feeding was 11 days. At 12 months, five patients remained tracheostomy-dependent and nine required enteral feeding. Median FOIS score at 12 months was 6, indicating near-normal oral intake. Conclusion: In carefully selected patients with recurrent laryngeal and hypopharyngeal SCC, salvage partial laryngectomy provides acceptable oncologic control with meaningful functional preservation. SPL represents a viable organ-preserving alternative to total laryngectomy in experienced centres.
