Thyroid Metastasis from Clear Cell Renal Cell Carcinoma with Concurrent Papillary Thyroid Carcinoma

Aims: To report a rare case of thyroid metastasis from clear cell renal cell carcinoma (RCC) four years after nephrectomy, occurring concurrently with papillary thyroid carcinoma (PTC). Methodology: A 60-year-old female with prior right radical nephrectomy for clear cell RCC presented with an enlarging anterior neck mass. Thyroid function tests were normal. Ultrasonography revealed multiple nodules. Total thyroidectomy was performed, followed by histopathologic and immunohistochemical evaluation. Results: Pathology revealed classic PTC (3.8cm) confined to the left lobe and isthmus with lymphovascular invasion. A separate 0.8cm clear cell neoplasm was identified. Immunohistochemistry demonstrated diffuse CD10 positivity and negativity for thyroglobulin and GATA3, confirming metastatic clear cell RCC. The thyroid was the only detected metastatic site. The 4-year interval from nephrectomy to thyroid metastasis was shorter than typically reported. Conclusion: This case underscores the need for sustained oncologic vigilance in patients with prior RCC. Even years after nephrectomy, metastatic disease may masquerade as primary thyroid pathology. Coexistence with PTC further obscures diagnosis, reinforcing the indispensable role of definitive surgical pathology in guiding prognosis and management.
