Aims Practice-embedded research is critical to improving head and neck cancer (HNC) outcomes, however, specialist supportive care clinicians are constrained by limited time, infrastructure and training. We describe the development and five-year evaluation of the ENHANCE Research Program, a clinician-researcher-led initiative to build research capacity and capability for translational impact. Methodology Program outputs and impact were evaluated (July 2020 to June 2025) using the Research Capacity Development for Impact Framework and the Association of Australian Medical Research Institute Research Impact (AAMRI) Framework. Bibliometric and program record data were analysed descriptively. Results ENHANCE comprised 22 members (7 disciplines, 46 studies, 2671 participants). Key outputs across AAMRI domains include: Health and social impact: Research addressed key clinical (eg dysphagia, laryngectomy, lymphoedema, malnutrition, tracheostomy, trismus) and system-level needs across the care continuum, including guideline development and implementation, improved supportive care access and treatment completion; Economic impact: reductions in unplanned admissions, length of stay, healthcare costs and one patent; Advancing knowledge: Annual publications increased from 12 to 36 (+200%) contributing 116 publications (57% Q1; 6% top 1% journals; 19% top 10% most cited), 1279 citations and FWCI 1.2. Dissemination included 74 presentations (57% international; 36% invited), 47 abstracts accepted (66% oral), achieving 14 awards/prizes; Capacity-building was evidenced by 2 PhD completions and 3 enrolments, formal mentorship and clinician-researcher leadership securing $3.36M in competitive funding (Category 1–3: 17%, 33%, 50%). Conclusion A structured clinician-researcher-led model can support a high-quality, practice-embedded and sustainable research program within routine care to deliver measurable translational impact for patients with HNC.
