Aims: To increase the number of laryngectomees who speak hands-free, we aim develop an implantable prosthetic device anchored to the manubrium sterni. As a first step towards its design, insight into valve motion during head movement exercises and speech is required.
Methodology: This cross-sectional study was conducted at the Department of Head and Neck Surgery of the Netherlands Cancer Institute in Amsterdam, the Netherlands. We included fourteen laryngectomees aged 50–74. Head and valve motion with and without valve fixation was assessed during head and neck movement exercises, reading and loud phonation. Discomfort was rated on a numerical rating scale.
Results: Valve motion without fixation was greatest during rotation and extension. During those exercises, median motion was most pronounced in right, left and anterior directions, measuring -12.2 [-15.6 – -9.5], 11.1 [8.4 – 13.9] and 10.2 [6.6 – 13.5] mm, respectively. During speech, the greatest motion was 21.5 [14.9 – 24.2] mm anteriorly. Valve fixation consistently led to restriction of head mobility.
Conclusion: A novel device providing valve fixation without restricting head mobility, should accommodate 29.2 and 7.9 mm along the right-left and superior-inferior axes, respectively, and up to 13.5 mm anteriorly, whilst simultaneously hindering anterior motion exceeding that value.
